Soon, Shelton carries out his threats from his cell in prison, organizing a series of murders spectacularly evil that can not be predicted or prevented. Cuando los asesinos son detenidos, el caso le es asignado a Nick Rice (Jamie Foxx), un prestigioso fiscal de Filadelfia. Nick propone a uno de los sospechosos la posibilidad de conseguir una sentencia leve a cambio de que testifique en contra de su cómplice. Diez años después, el hombre que consiguió evitar la acusación de asesinato es encontrado muerto y Clyde Shelton admite fríamente su culpa. Posteriormente, le hace una advertencia a Nick: O bien pone remedio al defectuoso sistema judicial que no hizo nada por su familia, o los principales implicados en el juicio morirán. Muy pronto, Shelton lleva a cabo sus amenazas desde su celda en la cárcel, organizando una serie de asesinatos espectacularmente diabólicos que no se pueden predecir ni evitar. Filadelfia es presa del pánico cuando los prominentes objetivos de Shelton son asesinados uno tras otro y las autoridades son incapaces de poner fin a su reinado de terror. Sólo Nick puede terminar con los asesinatos, y para lograrlo debe burlar a ese brillante sociópata en un angustioso enfrentamiento de voluntades en el que hasta la menor equivocación supone la muerte. Con su propia familia ahora en el punto de mira de Shelton, Nick se ve inmerso en una desesperada carrera contra el tiempo enfrentándose a un adversario mortal que parece que va siempre un paso por delante de él.
