I had to do an additional step after extracting quake.exe of also unzipping the "a" and "b" folders before I could get setup.bat to work.
After unzipping all folders setup.bat worked great!

@blaymoric actualy, the game was modded, some genius had this idea to put weapons at the start of each level.. you can freaking get them with cheats, what's the point with them?

Downloaded, scanned with MSE and Malwarebyte, no issues. Unpacked it, scanned again, no errors. Ran the setup which uncompressed everything, ran both virus checks again and no issues at all. Not sure what Crazyinsanepenguin in on about, but using the latest virus updates for Dec 2013, there were no issues, and COMODO firewall didn't give any warnings.
